Answer (since I'm really sure you wanted to know)

Mary had had "had" where John had had "had had"; "had had" had had the teachers approval.

It's not like I got it the first time either.

Also, who in the world writes a sentence like that in the first place? If I were the editor I'd cross it out and write above it "Readers will get confused. This will cause them to feel uncomfortable and intellectually challenged. They will not like this feeling so they will set down the book and not recommend to others that they should buy it. As a result you will not make much money so you should just omit this sentence."
... or you know, something to that extent.
